    Well I went ahead and insalled the phpFormGenerator script in Fantastico, created the tables, and successfully connected to the MYSQL database that was created as part of the script install. I can see the form with file manager, but how to access it from the web site? Help!

    Whenever you install something in fantastico it should provide you with the link to the script on your site....

    You might want to re-install it and take a look at the on-screen information for how to access it.

    If the script is in /home/cPanelusername/public_html/forms/form.php

    that would translate to a web address of:

    domain.com/forms/form.php
